Unless you are hosting your own website, you should disable this service or even uninstall it. Most likely your SMTP is wide open and used for spam relay. However, if its hitting the roof again then try disabling/stopping each of the IIS services (FTP, SMTP, NNTP, WWW) one at a time until only IIS Admin Service is left. Disable the SMTP service and everything should be fine.
